Reinstatement Success Activities

You must complete the required activities before the Reinstatement Committee can consider your reinstatement to the University. Incomplete applications will not be considered (directions for completion). Deadlines for application occur on the 18th of each month and final deadlines are as follows: fall term, July 18; winter term, November 18; and spring/summer term, March 18.  The committe meets once a month excluding August, December, and April. A complete packet will include:

  1. your reinstatement application, (application form);
  2. the personal statement outlined on the application, including the following:
    • evidence of progress while away from the University
    • documentation of extenuating circumstances (Do not include original documents, submit copies)
    • a realistic assessment of what went wrong
    • specific steps you plan to improve your academic performance
  3. the web site activities marked with an " ** " outlined below;
  4. an unofficial transcript from WSU and from any other institution that you have attended since leaving WSU; and
  5. evidence of attendance at a Academic Success Center workshop. (Workshop topics can be found at success.wayne.edu/workshop_list.php). (verification form)

Review each of the topics below and be prepared to write about what you have learned related to the material.  The  "Learning Styles Reflection" and Study Skills Inventory results must be submitted with your reinstatement packet. Your personal statement and responses to the required web site activities should be word-processed. Handwritten materials will not be accepted.  The committee's decision will be communicated via the email address you provide on your application. Make sure it is in working order.

Please note that this appeal does not affect your financial aid status.  You may also need to submit a separate appeal for financial aid.

Read the introductory information then select "Take the Inventory" and answer the questions onlineHit the "Submit Query" at the bottom of the page.  This will give you your learning style.  This inventory will help you identify your learning strategies. Write down your results and go to the next link on this page to discover ways to utilize your learning style. It might be helpful to bookmark this page so that you can return to it easily as you go through each step of this activity.

Using the results of the activity above think about how your learning style affected your previous course work,  how you will apply your learning style to ensure academic success in the future, and answer the 6 questions provided. Submit the competed responses with your reinstatement packet.

Assess Your Study Skills

**Study Skills Inventory 

The purpose of this questionnaire is to help you get information about how you study right now. As you examine the results, you will discover your areas of strength and of weakness. The results will have meaning only if you are honest and respond as accurately as possible. For each statement, determine if that statement is true or false as it applies to you. If a statement is true about you, click on the button below that statement corresponding to true. If a statement is false as it applies to you, click on the button below that statement corresponding to false. Be sure to answer true or false for each statement. Answer carefully so that you get accurate information. Once you are done, click on the "continue" button at the bottom of the page to determine your strengths and weaknesses. Be sure to include a copy of your results with your reinstatement application packet.
